PASSPORT CLINIC & OPEN HOUSE IN PORTAGE
As your Member of Parliament, one of my top priorities is to remain accessible and assist you in navigating the services offered by the Government of Canada. That's why I'm excited to announce that I'm hosting a Passport Clinic and Open House in Portage la Prairie:

Saturday, February 22nd
Passport Clinic: 10AM - 3PM
Open House: 3PM - 5PM

If you're applying for your first passport or need assistance with a renewal, this clinic is a great opportunity for you. We’ll have a photographer available (for a fee), and my team will review your application and mail it in on your behalf, making the process as easy and convenient as possible.
​
Please take a moment to review the checklist below which outlines all the necessary instructions, how to pay for your passport photo, and the accepted forms of payment for your passport application.
​
Passport Applicants MUST:
(1) Be a resident of Portage-Lisgar
(2) Book a timeslot by calling my office at 1-204-325-1540 or emailing Branden.Leslie@parl.gc.ca.
(3) Read and comply with all information on the passport checklist below.

And if you don’t need passport assistance, no worries! I’ll be hosting an Open House immediately after the clinic, from 3 PM to 5 PM, at the same location. Feel free to stop by for a coffee and chat about anything that’s on your mind.​

Passport Clinic Checklist

Please comply with the information on this checklist and make sure that you have all required documents before arriving at the clinic. If you are unsure about any documents, contact our office prior to the clinic to avoid any delays.

For General (New) Adult Applicants​*
​
Please ensure you bring ALL the following items:​​

Photocopy of your Government-Issued Photo ID with Signature

Photocopy of your Driver's Licence or another valid ID with your photo and signature.​

Original Birth Certificate or Canadian Citizenship Card
-
Must be the original document.​

Guarantor
-
​A person who has known you for two years and has a valid Canadian Passport (the guarantor must be at least 18 ​years of age).

The application must be completed before arriving at the clinic. ​

* For those applying using their married surname, copy of proof of marriage is required.
​
If you do not bring a guarantor, they must complete all three of the following:

Fill & Sign the Guarantor Section on the Application​
-
Complete and sign the section that requests your guarantor information.​

Sign the Back of One of Your Passport Photos
-
Their signature must appear on the back of one of your passport-sized photos. ​

Sign and Date a Photocopy of Your Driver's Licence or Government-Issued Photo ID

They must sign and date both the front and back of the photocopy of your ID. ​

*You can renew your passport if it has been issued in the last fifteen (15) years and you were at least 16 years old.
